Health coaching to change behavior and reduce the risk of stroke
Health coaching can help your patients address the modifiable risk factors for stroke and chronic conditions. AllWays Health Partners members have access to free 1x1 support from our health coaches. These coaches are trained to help members get and stay motivated to eat better, get more exercise, quit smoking, and more.
If you're caring for an AllWays Health Partners member who's interested in health coaching, please tell them to call the number on the back of their member ID card and ask to speak with a health coach.
