FAQ: What to know if your patient is involved with Your Care Circle
The unique Your Care Circle program is founded on our belief that an “all hands on deck” approach can help our highest utilization members reach their individual goals and improve their quality of life.
What is the Your Care Circle program?
Your Care Circle (YCC) is a free care management program for our members with the highest utilization. Our goal is to effectively address a member’s unique bio-psycho-social health care needs to improve their overall quality of life. We increase engagement with these historically hard-to-reach individuals through member-centered and community-based strategies. The dedicated, interdisciplinary YCC care team is prepared to meet with members face-to-face in nearly any location.
How does the Your Care Circle program work?
Each member is assigned a care team, or Care Community, who works with them to develop an individualized care plan with goals such as enhancing access to appropriate care and improving quality of life. Each Care Community consists of a medical care manager, a behavioral health care manager, social care manager, community health worker, and a clinical support coordinator. When appropriate, the Care Community can expand to include a nutritionist, a medical director, pharmacist, rehabilitation specialist, or addictions/recovery coach.
What are the goals of the Your Care Family program? How does it help my patient?
- Improve member health outcomes and quality of life.
- Reduce emergency room and acute hospital admissions/readmissions through enhanced provider coordination and transition of care for members.
- Improve adherence with important primary preventive care and screenings to decrease the occurrence, emergence, or worsening of medical and behavioral health disorders.
- Collaborate with in-network practitioners to build a treatment plan that the member is comfortable with while also adhering to evidence-based guidelines and standards of care.
- Identify and arrange for services not typically covered under a member’s health insurance benefits such as: smartphone access, medically-tailored meals, and community health workers to assist or escort members to appointments.
What expectations does Your Care Circle have of me?
Our goal is to address the unique needs of each member through a comprehensive team approach—and to do so with important input from providers. You may hear from one of the care managers to help with coordination of care and/or services. We believe that with an “all hands on deck” approach, we can help members reach their individual goals and, in turn, improve their quality of life.
