Have any of your patients recently received a text message AllWays Health Partners? Maybe they’ve gotten a message about a well child visit or about diabetes care. These messages are part of our initiatives to engage patients and make it easy for them to be proactive about their own care.
Last week, we posted about our efforts to help reduce unnecessary emergency department visits hospital readmissions, which are part of our efforts to manage members with emerging risks. That program, along with the texting program, are part of our total population health initiatives.
The largest population health group is, of course, the one dedicated to keeping members at their healthiest. This category includes popular programs like health coaching, a 24/7 nurse advice line, and an interactive wellness portal. In additon, we’re always looking for new ways to engage our members and give them the information they need to make good choices about their care.
Hello, it’s AllWays Health Partners
AllWays Health Partners now sends text messages to our members in English or Spanish. These clear, simple messages encourage health and wellness in key areas like asthma, well child care, adolescent well care, anti-depressant medication management, postpartum care, and diabetes. The text campaigns encourage members to see their PCP when critical clinical services are due, and they educate members about the benefits available to them. The messages also provide links to health information and give members the opportunity to request care management support. Health Coaches follow-up with members who request additional assistance.
Of course, we have protocols in place to protect member privacy and to allow them to opt-out of the text messages. If members don’t want to receive texts, we offer them the option to get information and reminders by mail.
